The intent of this letter is to dispute the following medical account that appears on my credit report. Please provide me with the name and address of the {doctor name/hospital, etc.}, the date and type of service, and to whom the service was provided.
If you can furnish this information, I also would need the name of the person/entity providing this data, and the manner in which it was provided in order that I may pursue additional legal remedies. If you can't furnish the information requested, I expect a prompt removal of this medical record.
