Medical dispute letter for credit reporting agencies

Independent health care providers and hospital billing departments often make mistakes, especially if your name is rather common. In case if your credit report shows a medical account or two, that you want to dispute, here is the medical dispute letter you need to send to credit reporting agencies.

To whom it may concern,

The intent of this letter is to dispute the following medical account that appears on my credit report. I have neither knowledge nor records of account # {XXXXXXXX} from {doctor name/hospital, etc.} on my report # {XXXXX}. Please provide me with the name and address of the {doctor name/hospital, etc.}, the date and type of service, and to whom the service was provided.

If you can furnish this information, I also would need the name of the person/entity providing this data, and the manner in which it was provided in order that I may pursue additional legal remedies. If you can't furnish the information requested, I expect a prompt removal of this medical record.
