Credit report header - fist paragraph

• name of credit report service used by your loan officer
• name of company that ordered your report
• price paid by ordering company
• date when it was ordered
• file number - of the credit reporting agency - optional
• name of a person who ordered report

Credit report header - second paragraph

• your name and your co-applicant name if any
• social security numbers
• current address
• date of birth and length of present address are optional

Credit report header - third paragraph is optional

• employer name, address for you and co-applicant if any
• how long you are employed there
• position held
• income
• what company verified your employment
